Workshop Registration & Cancellation Policies: |

If you register by fax or hard copy, please send one registration form per attendee to avoid registration errors.

Please include credit card information or enclose a check payable to U.S. Heritage Group

All fees must be paid in U.S. dollars.

Full payment is necessary to secure your enrollment.

Checks must be payable on a bank located in the United States.

Registration fees must be paid in advance of the workshop.

Upon receipt of the completed form and full payment, we will forward further information.

Please note that your registration is not confirmed until you have received written notification from U.S. Heritage Group, Inc.

Class size is strictly limited, so early registration is encouraged.

We recognize that registrants may find it necessary to cancel their reservations.The participant may reschedule to another workshop at no additional cost, provided that the request is in writing and made 48hrs in advance. All other cancellations are non-refundable.

| Travel Options: |
- Air: Fly into Chicago O'Hare airport or Midway the night before the workshop. Depart the last day of the workshop after 7:00 PM.

- Car: U.S. Heritage Training Center is located approximately five miles west of Wrigley Field. To reach us from the Kennedy Expressway (I-90):
- Exit at Addison Street; head west on Addison.
- Turn south onto North Kostner Avenue, which is one block past the intersection of Milwaukee Avenue and West Addison Street.
- Our address is 3516 North Kostner Avenue.

- CTA Blue Line train: Exit at Addison Street and catch the westbound 152 Addison bus. Get off at Kostner Avenue (the first stop after the intersection of Milwaukee Avenue and Addison Street.) - Metra trains: The Metra Milwaukee District North Line train (service to Fox Lake) has a nearby stop, Greyland. From the Greyland station, walk south approximately three blocks on Kilbourn Avenue; cross Addison and go left (east) on Addison and walk one block til you reach Kostner Avenue. - Lodging:We recommend that you stay at the Best Western at O'Hare, which is located at 10300 W. Higgins Road, Rosemont, Illinois.
- U.S. Heritage Group has negotiated a discounted price for workshop participants. To make a reservation, call the hotel directly at (847) 296-4471 and ask for the U.S. Heritage Workshop price.
- Transportation between the hotel and the training center is providedeach day. hotel shuttle bus service to O'Hare Airport is also provided.

Workshop Facilities, Schedule, & Amenities

Facility: 12,000 sq. ft. on two levels with heated classroom and hands-on workshop complete with practice masonry walls, a testing laboratory with national sand library, and a mortar production facility.
- Schedule 8.00 AM - 4.00 PM
- Food: Coffee, tea and rolls provided each morning. A hearty lunch, snacks, and complimentary bottled water provided each day. Dinner is not included.

| Miscellaneous |
Our workshops include hands-on activities, so please wear comfortable work clothing. Students are encouraged to bring personal hand tools or project material samples to the class. The facility is heated but we recommend that you dress warmly in the winter when the workshop can still be a little cool. Safety glasses are provided. Work gloves are not necessary but encouraged.
